Open Monday-Friday, excluding holidays, 9:00am-3:30pm

Cost
Online: $15 + $8.30 mailing and service charge
In-person: $15 for each copy after the original

Payment
Acceptable payment methods include: credit card, debit card, check
or money order (made payable to the NYC Department of Health and Mental Hygiene).
No cash payments accepted.

When applying for a Social Security Card:
Please remember:
 To have original documents proving: age, citizenship, and identity
 To have a completed application
*Take your completed application into your nearest Social Security Office

Do You Need A NYS Non-Driver's ID?

There are a few things that you need to be
prepared for when applying for your first
NYS Non-Driver’s Identification Card:

 Complete the MV-44 form
 Have proof of your name
 Have proof of your date of birth
 Have your social security card
 Be ready to pay any fees that may
apply

Locate the nearest Department of Motor
Vehicle Center and bring these documents
in!

Do You Need To Register To
Vote?

To register to vote in the City of New York,
you must:

 Be a citizen of the United States
(Includes those persons born in Puerto
Rico, Guam and the U.S. Virgin
Islands).
 Be a New York City resident for at
least 30 days.
 Be 18 years of age before the next
election.
 Not be serving a jail sentence or be on
parole for a felony conviction.
 Not be adjudged mentally incompetent
by a court.
 Not claim the right to vote elsewhere
(outside the City of New York).
Although you can register any time during
the year, your form must be delivered or
mailed at least 25 days before the next
election for it to be effective for that election.
